Oestradiol Blood Test in Australia

An oestradiol blood test measures the main active oestrogen, reported in pmol/L — it swings roughly tenfold across a normal menstrual cycle, falls sharply after menopause, and in men is a small but functionally important hormone made by converting testosterone through the enzyme aromatase.

What an oestradiol blood test measures

Oestradiol is the most biologically active of the three human oestrogens, produced mainly by the ovaries in women and by aromatisation of testosterone in fat, brain, bone and testes in men.

There are three oestrogens: oestrone (E1), oestradiol (E2) and oestriol (E3). Oestradiol is the one measured in almost all clinical situations because it is the dominant, most potent form during reproductive life. Oestrone becomes relatively more important after menopause, and oestriol is essentially a pregnancy hormone.

In women of reproductive age, oestradiol is produced by the granulosa cells of the developing follicle. It rises through the follicular phase as the follicle matures, peaks sharply just before ovulation — the surge that triggers the LH surge — falls, then rises again more modestly during the luteal phase from the corpus luteum. That trajectory is why cycle day is not a nicety but the precondition for interpreting the number at all.

In men, oestradiol comes almost entirely from converting testosterone via aromatase, an enzyme concentrated in adipose tissue. It is not a waste product. Oestradiol in men maintains bone density, modulates libido, and contributes to the negative feedback that regulates LH — men with genetic aromatase deficiency have low bone density and continue growing into adulthood despite normal testosterone.

Australian laboratories report oestradiol in picomoles per litre. American articles quote pg/mL. Multiply pg/mL by 3.67 to convert: a US figure of 30 pg/mL is about 110 pmol/L.

The assay caveat above deserves emphasis, because it produces a great deal of confusion online. Immunoassays are calibrated for the concentrations seen in cycling women. Used at male or post-menopausal concentrations, they cross-react and read inaccurately — frequently too high. Men comparing their immunoassay oestradiol to a threshold quoted from an LC-MS/MS study are not comparing like with like.

Oestradiol sits in both the female cycle panel and the male hormone panel, read against different companions in each.

What a high oestradiol can indicate

High oestradiol reflects the cycle phase in women, and in men usually reflects increased aromatisation from higher body fat, exogenous testosterone, or liver disease.

In a cycling woman, an oestradiol of 900 pmol/L is unremarkable at mid-cycle and unexpected on day 3. A raised early-follicular oestradiol can suppress FSH and mask reduced ovarian reserve, which is exactly why the two are measured together on day 2–5. Persistently high levels outside the cycle pattern raise questions about ovarian cysts, ovarian stimulation, or, rarely, an oestrogen-producing tumour.

In men, the commonest reasons are higher adipose mass — fat is where aromatase lives — and exogenous testosterone or anabolic steroids, which supply more substrate for conversion. Liver disease raises oestradiol because the liver clears it, and alcohol contributes both directly and through liver effects. Some medicines interfere with androgen–oestrogen balance.

The clinical correlate men usually arrive with is gynaecomastia — glandular breast tissue rather than fat. That warrants a proper work-up including testosterone, LH, hCG, prolactin, thyroid and liver function, because a small number of cases reflect something that needs finding. It is not a self-managed number.

What a low oestradiol can indicate

Low oestradiol in women reflects menopause, ovarian insufficiency, or suppression of the reproductive axis; in men it is uncommon and is usually seen with low testosterone or over-suppression.

In a woman under 40, a persistently low oestradiol with a high FSH suggests primary ovarian insufficiency and needs specialist assessment — this is a diagnosis with implications for bone and cardiovascular health, not only fertility. A low oestradiol with a low or normal FSH points instead to hypothalamic suppression: low energy availability, heavy training load, significant weight loss, chronic illness, or hyperprolactinaemia.

Around menopause the picture is not a smooth decline. Perimenopausal oestradiol fluctuates unpredictably and can be high one month and low the next, which is why Australian guidance is that menopause in women over 45 is diagnosed clinically on symptoms and cycle history rather than on hormone levels. A single normal oestradiol does not exclude perimenopause.

In men, low oestradiol usually accompanies low testosterone, since there is less substrate to convert. Very low levels are associated with reduced bone density and, in some men, with poorer libido — a reminder that oestradiol in men is not something to be minimised.

What affects an oestradiol result

Cycle day dominates in women; body composition, alcohol, medicines and assay method dominate elsewhere.

  • Cycle day — the single largest determinant in a cycling woman. Always record the first day of your last period.
  • Hormonal contraception suppresses endogenous oestradiol; some formulations also interfere with the assay.
  • Menopausal hormone therapy raises measured oestradiol, with the degree depending on route and preparation.
  • Body fat: adipose tissue is the main site of aromatase in men, so oestradiol tends to rise with BMI.
  • Exogenous testosterone or anabolic steroids raise oestradiol by supplying more substrate.
  • Alcohol and liver disease raise oestradiol by reducing clearance.
  • High-dose biotin supplements interfere with immunoassays — stop 48–72 hours before testing.
  • Assay method: immunoassay versus LC-MS/MS materially changes results at low concentrations.
  • Pregnancy raises oestradiol enormously; results in pregnancy are interpreted against pregnancy-specific ranges.

Who should consider an oestradiol test

Oestradiol is ordered as part of a cycle, menopause or male hormone assessment, rather than as a standalone screening test.

  • Irregular, absent or changing cycles, alongside FSH, LH and prolactin.
  • Fertility assessment — the day 2–5 oestradiol is needed to interpret FSH correctly.
  • Suspected premature ovarian insufficiency in a woman under 40.
  • Perimenopausal symptoms, understanding that levels fluctuate and clinical assessment leads.
  • Men with gynaecomastia, low libido or unexplained low testosterone.
  • Men with a history of testosterone or anabolic steroid use, where aromatisation is relevant.
  • Monitoring directed by an endocrinologist or fertility specialist.

Frequently asked questions

What is a normal oestradiol level in Australia?
Indicatively: 90–400 pmol/L in the early follicular phase, 400–1,600 pmol/L at the mid-cycle peak, 180–1,000 pmol/L in the luteal phase, under about 100 pmol/L after menopause, and 40–160 pmol/L in adult men. Intervals are assay-specific and your report's own range applies.
When in my cycle should oestradiol be tested?
Day 2 to 5 for a baseline, counting the first day of full flow as day 1. That is when it is measured with FSH, because a high early oestradiol can suppress FSH and hide reduced ovarian reserve.
Why is oestradiol measured in men?
Because men convert testosterone to oestradiol via aromatase, and that oestradiol maintains bone density, contributes to libido and helps regulate LH. It is measured where there is gynaecomastia, unexplained low testosterone, or a history of testosterone or anabolic steroid use.
Does a normal oestradiol rule out menopause?
No. Perimenopausal oestradiol fluctuates widely and can be normal or even high between low months. In women over 45, Australian guidance is that menopause is diagnosed clinically from symptoms and cycle history rather than from hormone levels.
Why is my result in pmol/L when US articles use pg/mL?
Australian laboratories use SI units. Multiply pg/mL by 3.67 to convert — so 30 pg/mL is about 110 pmol/L.
Do I need to fast for an oestradiol test?
No. Fasting is not required. Morning collection is preferred in men and where other hormones are measured on the same sample.

Can I test oestradiol on the pill?
You can, but combined hormonal contraception suppresses your own oestradiol production, so the result reflects the medication rather than your ovarian function.
